Every appeal shall be in writing on forms approved by the board and furnished by the building inspector.

Every such appeal shall be filed with the building official at least ten days prior to being considered by the board.

Every such appeal shall indicate the code provision or provisions concerned, the nature of the appeal, the type of material or construction intended to be used and shall be accompanied by such drawings, plans, information or other materials deemed appropriate by the board.
