All persons desiring to operate automobiles for hire in the city shall make application for a certificate, which shall contain the following information:
A. All of the information required by Section 14.12.010;
B. A statement indicating the location of the garage from which the automobile will be dispatched;
C. A general statement that the applicant has complied or prior to the commencement of operations will comply with all of the terms and conditions of this chapter.
