A. The name, address, and telephone numbers of the permit applicant, organization sponsoring the activity (if any) and person who shall be in charge of the activity;

B. Park, facility or area thereof requested;

C. Any additional park equipment or personnel requested;

D. The type of activity planned;

E. The starting and finishing time of activity;

F. The number of persons expected to attend the activity;

G. The type of equipment or structures (if any) which shall be erected or placed on park property;

H. Whether the activity will involve amplified sound and the nature thereof;

I. Type and amount of insurance coverage for the event;

J. Any supplemental information the director shall find reasonably necessary to determine whether to approve the permit application pursuant to Section 13.34.090.
